The following areas of concern have not been addressed in the
proposal and must be included in the workplan to obtain approval by PHS/EHD.
PHS/EHD requires that the closure in place workplan include plans for closure of the UST, contamination,
sump and pipeline. The environmental risk analysis model of the contamination must use the 1986 high
groundwater measurement of 35 feet below ground surface as an input parameter.
Approval of the closure in place workplan will be granted by PHS/EHD with the condition that all of the
provisions outlined in the proposal will be met and all areas of concern described in this correspondence
will be resolved.
For additional information please contact Steven Sasson at (209) 468-3459.
Donna Heran, REHS, Director
Environmental Health Division
St ven Sasson, Senior REHS Margaret Lagorio, REHS
Site Mitigation Unit Lead Senior
SS
cc: Mike Golden, Division of the State Architect
cc: Brian West, West Associates Environmental Engineers, Inc.
cc: James Brathovde, CVRWQCB UST Section
A Division of San Joaquin County Health Care Services
