In the work plan, CRA <br /> proposes to clear to eight feet below surface <br /> grade around each <br /> monitoring well using an <br /> air vacuum truck or hand auger followed by removing the traffic.box from:each monitoring <br /> well; then the wells will be destroyed by over drilling theboreholes to total depth using <br /> eight-inch diameter hollow-stem augers. The boreholes will be grouted with Portland <br /> cement using a tremie pipe. . <br /> The. EHD approves the work plan as submitted for destroying monitoring wells MW-1, <br /> MW-2, MW-3, MW-4, MW-5S, MW-5D,. MW-6S, MW-6D, and MW-7D. . However, <br /> because MW- <br /> 8 has atwelve-inch diameter borehole MW-8 must be destroyed by over.drilling the borehole to total depth using twelve-inch or larger diameter hollow-stem <br /> augers.. <br /> By letter dated May 23, 2008, the EHD directed you to.provide certification in writing that <br /> all record fee title owners have been notified of your request for closure. This is in <br /> accordance with Section 25297.15,of the California Health and Safety Code. The EHD has. <br /> not received a list of all record fee title owners and their mailing addresses. If you are the <br /> sole property owner, provide a statement to ,this effect to the EHD. Please submit this <br /> I <br /> information at your earliest convenience so that the closure process is not delayed. <br /> The EHD has received a well destruction permit application and permit fee of$60 from <br /> Conestoga-Rovers & Associates and will issue the permit today. The EHD has been t <br /> informed that the fieldwork will take place June 4th through June 6th,2008. . <br />
