Before a NFAR letter can be provided to you, the <br /> monitoring wells at the site must be properly destroyed. <br /> Please submit a work plan to the EHD to destroy all on-site wells. Note that monitoring <br /> wells in area of contamination may only be destroyed by removing all of the well contents <br /> by drilling out the original borehole and filling it with grout. <br /> In addition to the above, please provide EHD certification in writing that all record fee title <br /> owners have been notified of your request for closure. This is in accordance with Section <br /> 25297.15 of the California Health and Safety Code. Submit a list of all record fee title <br /> owners and their mailing addresses. EHD will send notification to each title owner of the <br /> pending closure letter. If you are the sole property owner, provide a statement to this effect <br /> to EHD. A form is enclosed for your convenience. Please submit this information at your <br /> earliest convenience so that the closure process is not delayed. <br /> The EHD has received a well destruction permit application and permit fee of $89 from <br /> Conestoga-Rovers & Associates. The EHD is returning the check for $89 since the well <br /> destruction fee is $60. The EHD will not issue the permit until the EHD has received and <br /> approved a work plan for destroying the ten on-site monitoring wells. <br />
