a
I
PUBLIC HEALTH SERVICES '
SAN JOAQUIN COUNTY r.
ENVIRONMENTAL HEALTH DIVISION <
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor•Stockton, CA 95202
209/468-3420 .
FILE UOPY
RUSSELL HULME
PROFESSIONAL MAINTENANCE COMPANY INC AUG Q 2 1999
899 PARK AVENUE
SAN JOSE CA 95126
i
i
RE: 510-560 N. Main Street
Manteca,CA i
San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) has
reviewed pages from the Ninyo & Moore report"Project No. 400032-01" of which figure 2 is
dated 8/98 for the above referenced site. The report states that three monitoring wells were
observed in the southern portion of the property and that there was no documentation of well
abandonment in the SJCEHD case file. The report recommends destruction of those
monitoring wells.
The PHS/EHD Local Oversight Program (LOP)file contains permits for destruction of two
wells in September 1990 and six wells in March 1`994. Monitoring wells 1 and 3 were
destroyed during excavation work in September 1990. Monitoring wells 2,4, 5, 6, 7 and 8 `
were destroyed by pressure grouting cement into the well casing and filling the christy box
with the cement grout. Although the christy boxes with secured lids remain in.the southern
portion of the site, there are no monitoring wells remaining. This information is also
documented in the case closure summary that is in the PHS/EHD LOP file focthe site, which
indicates that 8 monitoring wells were destroyed and that 0 remain.
i
PHS/EHD staff went to the site on July 27, 1999 to verify this information. The lids of the
christy boxes have been welded to the rim of the box. MW8 was only welded in one spot so j
the lid could be removed. PHS/EHD staff verified that there was cement inside the christy
box.
PHS/EHD appreciates Ninyo & Moore's concern for abandoned monitoring wells, but is sure
that all monitoring wells at this site were properly destroyed under PHS/EHD permit and
inspection.
If you have any questions contact me at(209)468-3449.
Donna Heran, REHS, Director
Environmental Health Division -
J
Margaret agorio,'Supervising REHS
Site Mitigation Unit
i -
A Division of San Joaquin County Health Care Services
