Cummings and Associates (JPCA) is pleased to offer this proposal, in response to the work plan items requested by San Joaquin County Environmental Health Division (SJCEHD) in letters to you dated December 5, 2dOO, March 8, 2001 and May 22 2001 for the real property located at 4075 East Main Street, Stockton CA. This workplan includes the requirements for the quarterly monitoring of the wells, for four quarters, as required by the San Joaquin County Environmental Health Division (SJCEHD) in their letter dated May 22 2001. This workplan will be submitted to the SJCEHD for their approval . BACKGROUND The parcel is approximately rectangular. With,.approximately 150 .—feet on Oro-Street north to the lot's rear border; then approximately 150 feet along the rear fence; then approximately 120 feet southerly to East Main Street and then approximately 160 feet 'easterly to the NW Corner of the intersection of Oro and Maim Streets. Figure 1 is a site location map. The following proposal is based on the UST removal report (June 6, 1995 ) from the site. The prior Phase II Report from JPCA and the above identified letters from SJCEHD to you. Prior work indicated no or minimum levels of hydrocarbon contamination in both the soil and groundwater at ,the MW-5 location. The contamination appears to be on the south and east portion of the subject site. k 1 h
