Box 388 <br /> Stockton, California 95201-0388 <br /> Subject: Submittal of Well Construction Permit Applications <br /> Beacon Station No. 474 <br /> 3440 East Main Street, Stockton <br /> Mr. Infurna: <br /> At the request of Ultramar Inc. (Ultramar), Horizon Environmental Inc. (Horizon) is <br /> forwarding the enclosed two Well Construction Applications (and attachments) for <br /> additional monitoring wells at the above-referenced site. Ultramar previously submitted the <br /> Work Plan for a Remedial Investigation (Horizon, June 1, 1998) and the Letter Addendum to <br /> Work Plan for a Remedial Investigation (Horizon, December 28, 1998) to the San Joaquin <br /> County Public Health Services - Environmental Health Division (SJCPHS-EHD). On <br /> January 19, 1999, Ultramar received approval of the proposed work from the SJCPHS-EHD, <br /> and wishes to proceed with the proposed scope of work at the site. The location of pertinent <br /> site features and the proposed soil borings and groundwater monitoring wells are shown on <br /> the rear sides of each of the Well Construction Applications. <br /> Two Well Construction Applications have been completed and are attached with this letter. <br /> One application has been submitted for the proposed onsite soil borings, groundwater <br /> monitoring wells, and vapor extraction wells. The second application has been submitted for <br /> proposed offsite groundwater monitoring well MW-10, which is to be located in the East <br /> Main Street right-of-way. On August 6 1999, Ultramar finally received an Encroachment <br /> Permit from the City of Stockton. <br /> A copy of the SJCPHS-EHD approval letter and the City of Stockton Revocable <br /> Encroachment Permit No. 99-13 are included with the Well Construction Applications. We <br /> understand that the appropriate fees are $89.00 per application. A check for the Well <br /> Construction Application fees of $178.00 has been enclosed with this letter. The drilling <br /> subcontractor will be V&W Drilling of Rio Vista, California. The proposed work has <br /> been scheduled to begin on Monday, October 18, 1999.
