RECEIVED <br /> San Joaquin County Environmental Health Department <br /> 1868 East Hazelton Avenue MAY 2 2 2014 <br /> Stockton, California 95205-6232 <br /> ENVIRONMENTAL HEALTH <br /> Subject: Well Destruction Report PERMIT/SERVICES <br /> Former Beacon Station No. 12474 <br /> 3440 East Main Street, Stockton, California <br /> Ms. McCartney: <br /> On behalf of Ultramar Inc. (Ultramar), Horizon Environmental Inc. (Horizon) is submitting <br /> this report documenting the destruction of 25 wells associated with the subject site (Site) <br /> located in San Joaquin County as shown on the enclosed Figure 1. In the attached 30 <br /> October 2013 correspondence, the San Joaquin County Environmental Health Department <br /> (EHD) confirmed the completion of the public participation period regarding the regulatory <br /> closure of the Site and directed Ultramar to proceed with the destruction of all wells related <br /> to the Site environmental investigation. Horizon submitted a Well Destruction Work Plan <br /> (Work Plan) dated 3 December 2013. The EHD reviewed and approved the Work Plan in <br /> the enclosed letter dated 8 January 2014. <br /> Fieldwork Planning <br /> Wells associated with the Site were located on the Site parcel and in City of Stockton rights- <br /> of-way as shown on the enclosed Figure 2. Located on the Site (APN 157-160-02) were <br /> monitoring wells MW-1 through MW-3, MW-6 through MW-21, vapor wells VW-1/2/3, <br /> VW-4/5/6, VW-7 and air sparge well AS-1. Monitoring wells MW-4 and MW-5 were <br /> located in the East Main Street and Broadway Avenue rights-of-way. <br /> Horizon retained: All Well Abandonment (AWA) of Sacramento, California (C-57 License <br /> No. 848359) to provide hollow-stem auger well destruction services; and Belshire <br /> Environmental Services (Belshire) of Foothill Ranch, California to provide solid-waste <br /> disposal services. Horizon obtained right-of-way Encroachment Permit No. 76753 from the <br /> City of Stockton Community Development Department (CDD) on 6 February 2014 and <br /> EHD well destruction permits Nos. SR # 69102 and SR # 69104 issued on 21 February <br /> 2014. Copies of the CCD and EHD permits are enclosed. <br /> Horizon updated the Site Health and Safety Plan to include well abandonment activities and <br /> notified Underground Services Alert (USA) to locate and mark locations of underground <br /> utilities. Property owners, CCD and EMD personnel were notified in advance of the <br /> fieldwork schedule.
