San Joaquin County Environmental Health Department DEC 0 6 2013 1868 East Hazelton Avenue Stockton, California 95205-6232 ENVIRONMENTAL HEALTH PERMIT/SERVICES Subject: Well Destruction Work Plan Former Beacon Station No. 12474 3440 East Main Street, Stockton, California Ms. McCartney: At the request of Ultramar Inc., Horizon Environmental Inc. (Horizon) is submitting this well destruction work plan for the subject site (Site) located in San Joaquin County as shown on Figure 1. The San Joaquin County Environmental Health Department (EHD) requested the work plan in a letter dated 30 October 2013. In an email dated 9 October 2013, the EHD directed that all on-site wells be destroyed by drilling out the wells and that the two off-site wells, MW-4 and MW-5 could be destroyed by pressure grouting. Wells associated with the Site are shown on Figure 2. Wells to be destroyed include 21 groundwater monitoring wells (MW-1 through MW-21), two multiple-completion vapor extraction wells (VW-1/2/3 and VW-4/5/6), one single- completion vapor well (MW-7) and one air-sparge well (AS-1). With the exception of wells MW-4 and MW-5, all wells are located on the Site property (APN 157-160-02). Wells MW-4 and MW-5 are located in East Main Street and Broadway (City of Stockton) right-of- way (ROW). One permit application shall be submitted for wells located on Site, and one permit shall be submitted for wells in the Stockton ROW. Well construction data and estimated grout volumes required to destroy the wells are summarized in the enclosed table. Well destruction work shall consist of the following tasks: Task 1 Select and schedule a C-57 licensed drilling contractor to provide well destruction services; obtain a City of Stockton encroachment permit for destruction of wells MW-4 and MW-5; obtain well destruction permits from the EHD; arrange for a roll-off bin to contain well cuttings and debris; contact USA to mark underground utilities in the areas of the wells; and update the Site Health and Safety Plan to include well destruction activities.
