(Horizon) is submitting this <br /> well destruction work plan for the subject site (Site) located in San Joaquin County as shown <br /> on Figure 1. The San Joaquin County Environmental Health Department (EHD) requested <br /> the work plan in a letter dated 30 October 2013. In an email dated 9 October 2013, the EHD <br /> directed that all on-site wells be destroyed by drilling out the wells and that the two off-site <br /> wells, MW-4 and MW-5 could be destroyed by pressure grouting. Wells associated with the <br /> Site are shown on Figure 2. <br /> Wells to be destroyed include 21 groundwater monitoring wells (MW-1 through MW-21), <br /> two multiple-completion vapor extraction wells (VW-1/2/3 and VW-4/5/6), one single- <br /> completion vapor well (MW-7) and one air-sparge well (AS-1). With the exception of wells <br /> MW-4 and MW-5, all wells are located on the Site property (APN 157-160-02). Wells <br /> MW-4 and MW-5 are located in East Main Street and Broadway (City of Stockton) right-of- <br /> way (ROW). One permit application shall be submitted for wells located on Site, and one <br /> permit shall be submitted for wells in the Stockton ROW. Well construction data and <br /> estimated grout volumes required to destroy the wells are summarized in the enclosed table. <br /> Well destruction work shall consist of the following tasks: <br /> Task 1 Select and schedule a C-57 licensed drilling contractor to provide well <br /> destruction services; obtain a City of Stockton encroachment permit for <br /> destruction of wells MW-4 and MW-5; obtain well destruction permits from <br /> the EHD; arrange for a roll-off bin to contain well cuttings and debris; <br /> contact USA to mark underground utilities in the areas of the wells; and <br /> update the Site Health and Safety Plan to include well destruction activities. <br /> Task 2 Disconnect remediation piping from connected wells; drill-out selected wells <br /> to total depth of the well casing(s) and to the diameter of the original bore <br /> hole(s); backfill borings with neat cement via tremie set to within 5 feet of <br /> total depth of the borings; top off neat cement backfill until stabilized within <br /> 5 feet below surface grade (bsg); emplace native, sandy or base material to <br /> within 1 foot bsg and restore the surface, as required for each well location. <br /> 4970 Winciplay Drive,#C5 • EI Dorado Hills,CA 95762 • (916)939-2170 • FAX(916)939-2172 <br /> I O.Box 5283 • Bakersfield,CA 93388 • (661)589-8389 • FAX(661)589-1456 <br />
