REVOCABLE PERMIT <br /> Ground Water Monitoring Well <br /> Permit No.: 96-04 <br /> Date: February 20, 1996 <br /> Location: 3440 E. MAIN STREET <br /> Permittee: ULTRAMAR INC. <br /> 525 WEST THIRD STREET <br /> HANFORD. CA 93230 <br /> In accordance with Ordinance 003-96, passed by the Council of the City of Stockton on January 22, <br /> 1996, a permit revocable at the pleasure of the Public Works Director of the City of Stockton is <br /> hereby granted to: <br /> ULTRAMAR INC. <br /> to install and maintain two (2) monitoring well at 3440 E. Main Street, Stockton, CA. <br /> This permit is granted under the following terms and conditions: <br /> 1. This revocable permit is granted subject to the Standard Conditions Applicable to Revocable <br /> Permits adopted by this City Council by Resolution No. 22,578 on December 5, 1960, and <br /> on file in the Office of the City Clerk of the City of Stockton. <br /> 2. The general location of each well shall be as shown on the map attached. In accordance with Section 1(h) of the "Standard Conditions Applicable to Revocable <br /> Permits," the permittee shall provide and keep in force during the term of the permit a policy <br /> of public liability insurance against liability for injuries to persons or property arising out of <br /> accidents attributable to the structure on City property, in amounts to be determined by the <br /> Risk Manager. The policy of insurance so provided shall contain a contractual liability <br /> endorsement covering the liability assumed by the Permittee by the terms of his permit and <br /> shall contain a provision that such policy may not be canceled except after thirty (30) days <br /> notice in writing to be given to the Risk Manager. The copies of these policies or certificates <br /> evidencing the same shall be filed with the Risk Manager of the City after approval of the <br /> City Attorney. <br />
