Main <br /> Manteca CA 95336 <br /> This letter is in reference to the meeting held on June 11, 1991, <br /> with the Customer Company representative, John Johnson, San Joaquin <br /> County Public Health Services, Environmental Health Division <br /> (PHS/EHD) staff and the Central Valley Regional Water Quality <br /> Control Board (CVRWQCB) staff. <br /> As discussed in the above referenced meeting, we are requesting <br /> that a work plan to define the extent of groundwater contamination <br /> be submitted to PHS/EHD and to the CVRWQCB by July 31, 1991. <br /> This work plan should include, but not be limited to the following: <br /> 1. Installation of a downgradient well or wells. <br /> 2 . A survey of all municipal, domestic, agricultural or <br /> other wells within 2000 feet. <br /> 3 . A pump or slug test to evaluate the effect of those wells <br /> on the groundwater beneath the above referenced site. <br /> 4. Current well surveying and gradient information. <br /> 5. A quarterly groundwater sampling plan. <br /> 6. A spoils disposal or treatment plan. <br /> 7. A site specific health and safety plan. <br /> A complete Problem Assessment Report (PAR) is to be submitted to <br /> this office and to the CVRWQCB by September 27, 1991. <br /> If the investigation and report follows all the established <br /> protocol, the groundwater is found to be non-detectable for <br /> hydrocarbon contamination, and acceptable rational is provided <br /> which explains what may have happened to the previously <br /> contaminated groundwater, this site will be evaluated for closure. <br /> A Division of San Joaquin County Health Care Services 10 <br />
