REVOCABLE PERMIT Ground Water Monitorina Well Permit No 99-09 Date April 7 1 9 ' Location 2701 WEST MARCH LANE ' Permittee ENVIRONMENTAL RESOLUTIONS INC 73 DIGITAL DRIVE, SUITE 10 NOVATO. CA 94949 In accordance with Ordinance 003-96, passed by the Council of the City of Stockton on January 22, 1996, a permit revocable at the pleasure of the Public Works Director of the City of Stockton is hereby granted to ENVIRONMENTAL BESOLUTIONS INC. ' to install and maintain nine (9) monitoring wells, at 2701 West March Lane, Stockton, CA This permit is granted under the following terms and conditions 1 This revocable permit is granted subject to the Standard Conditions Applicable to Revocable Permits adopted by this Cty Council by Resolution No 22,578 on December 5, 1960, and ' on fife in the Office of the City Clerk of the City of Stockton 2 The general location of each well shall be as shown on the map attached The precise location shall be determined in the field by the City of Stockton 3 The Permittee has posted security of$4,500 00 with the City of Stockton which will remain in effect during the life of this permit 4 In accordance with Section 1(h) of the "Standard Conditions Applicable to Revocable Permits,"the permittee shall provide and keep in force dunng the term of the permit a polioy of public liability insurance against liability for injuries to persons or property ansing out of accidents attnbutable to the structure on City property, in amounts to be determined by the Risk Manager The policy of insurance so provided shall contain a contractual liability endorsement covenng the liability assumed by the Permittee by the termsof his permit and shall contain a provision that such policy may not be canceled except after thirty (30) days ' notice in writing to be given to the Risk Manager The copies of these policies or certificate, evidencing the same shall be fled with the Risk Manager of the City after approval of the City Attorney
