1.0 - INTRODUCTION <br /> 1.1 Scope and Applicability of the Site Health and Safety Plan <br /> This Health and Safety Plan(HASP)has been prepared by ATC for the activities associated with the <br /> gauging and sampling of monitoring wells, and drilling and sampling of additional monitoring wells at the <br /> former 76 Service Station No. 11190,located at 1206 E.March Lane in Stockton,California(Site). <br /> The health and safety protocols established in this Plan are based on the ATC Employee Health and Safety <br /> Policy Manual,the Occupational Safety and Health Administration(OSHA)Regulations,past field <br /> experiences,specific Site conditions,and chemical hazards known or anticipated to be present from <br /> available Site data. The following Site Health and Safety Plan(HASP)is intended solely for use during <br /> the proposed activities described in the project documents and technical specifications. Specifications <br /> herein are subject to review and revision based on actual conditions encountered in the field during Site <br /> characterization activities. Such changes may be instituted by using the HASP List of Approved <br /> Amendments and/or Changes(see Appendix Q. <br /> Before Site operations begin, all employees,including subcontractors for ATC covered by this plan, <br /> involved in these operations will have read and understood this HASP and all revisions. All Site personnel <br /> have the authority to"Stop Work"if unsafe conditions are present or discovered during Site activities. <br /> Before work begins, all affected workers will sign the Heath and Safety Plan Acknowledgment Form(see <br /> Appendix Q. By signing this form,all individuals recognize the requirements of the HASP,known or <br /> suspected hazards, and will adhere to the protocols required for the project Site. <br /> 1.2 Historical Overview <br /> The site is the location of an operating Shell gasoline service station. The facility is bounded on the north <br /> by March Lane and the east by West Lane. Above-ground structures at the site consist of a single-story, <br /> convenience store building and a car wash structure. Three 10,000-gallon underground storage tanks <br /> (USTs)containing unleaded gasoline are located north and west of the building. Six product dispenser <br /> islands are centrally located on the property. <br /> 1.3 Visitors <br /> All visitors to the Site must be instructed about the hazards of the activities that ATC or its subcontractors <br /> are performing. All visitors must sign the ATC Visitors Log(see Appendix Q. <br /> 1.4 Subcontractor Activities <br /> All subcontractors used at the Site have been Pre-Approved in the ATC Subcontractor Prequalification <br /> System. Subcontractors include drilling companies,and utility locators. <br /> 2.0 - PROJECT ORGANIZATION <br /> All personnel and visitors who may enter work areas on this Site must comply with the requirements of this <br /> HASP.All Site personnel have the authority to"Stop Work"if unsafe conditions are present. The specific <br /> responsibilities and authority of management, safety and health,and other personnel on this Site are <br /> detailed in the following sections. <br /> 2.1 Site Safety and Health Officer (SSHO) <br /> The Site Safety and Health Officer(SSHO)has the responsibility and authority to develop and implement <br /> this HASP and to verify compliance. The SSHO reports to the Project Manager. The SSHO is on-site <br /> HASP Level III <br /> Rev.(September 17,2012) 5-1 <br />
