USA STATIONS #65 AT 2500 WEST LODI AVE. , LODI, AND #110 <br /> AT 2132 EAST MARIPOSA ROAD, STOCKTON. <br /> This letter is a follow up to 26 September 1988 phone <br /> conversations I had with your consultants Mr. Barry Marcus and <br /> Mr. John Bobbitt, of Applied Goesystems, regarding USA stations <br /> #65 and #110 respectively. They had received copies of my <br /> 1 September 1988 letter from you too late to comply with the <br /> requested dates for submitting Preliminary Reports on the subject <br /> sites, so time extensions were requested. I agreed to extensions <br /> provided that extensions for the Problem Assessment Reports were <br /> not requested. They agreed that there would be no additional <br /> extensions needed. The new schedule for submitting reports is as <br /> follows: <br /> SITE REPORT REQUESTED DUE DATE <br /> USA Station #65 Preliminary Report 17 Oct. 1988 <br /> Problem Assessment 30 Dec. 1988 <br /> USA Station #110 Preliminary Report 24 Oct. 1988 <br /> Problem Assessment 12 Dec. 1988 <br /> I have enclosed copies of the Tri-Regional Board Sampling <br /> Guidelines and Region 5 's Appendix A Reporting Requirements. <br /> Please follow the formats set forth in these guidelines. Reports <br /> are to be submitted to both Ms. Laurie Cotulla of the San Joaquin <br /> Local Health District and this office.
