-2- <br /> REPORT CONTENT <br /> Each report should be a complete entity. If the report lacks data as described <br /> below, such as pagination, it should be returned to the tank owner for completion <br /> prior to resubmittal for regulatory review. <br /> PRELIMINARY REPORT <br /> Name and address of site, tank owner, and contact person, if different <br /> Map showing site location relative to nearby landmarks (North is up) <br /> Sketch of the site showing nearby streets and properties <br /> Layout of al l tanks and piping systems properly labeled for content <br /> Results of tank tests or initial site work done <br /> Estimates of liquid quantity and composition leaked into the environment <br /> Local topography, geology, nearby streams, depth to ground water <br /> Proximity of private or municipal wells and irrigation wells <br /> Location of nearby septic tanks, underground public utility lines <br /> Distance to adjacent buildings and their water and sewer lines <br /> Proposed sampling locations and methodology <br /> Based upon this information, the LIA and Regional Board probably could accept the <br /> initial proposal to begin work. Once the dri fling has provided data, the consultant <br /> could then follow up the Preliminary+ Report with a letter describing the findings <br /> and any additional borings on the modified site map needed to define the problem. <br /> We could review this proposal quickly and either approve verbally, in writing, or <br /> discuss at a meetinq with the tank owner and consultant. The consultant would <br /> then proceed to obtain the necessary permits and continue the problem <br /> assessment. <br /> PROBLEM ASSESSMENT <br /> During Problem Assessment, while the drilling equipment is on-site, boreholes and <br /> monitor wells should be completed as necessary to define the soil and around <br /> water problem. We can provide verbal authorization for additional drilling on-site. <br /> Off site drilling may require a slowdown but hopefully not a cessation of <br />
