Box 1839 � ? <br /> Santa Maria, CA 90406 <br /> RE: USA Service Station #110 <br /> 2132 Mariposa Road <br /> Stockton, CA 95205 <br /> A report of preliminary site assessment prepared by Gettler-Ryan, Inc. , has been <br /> submitted to this office and reveals groundwater contaminated with gasoline consti- <br /> tuents present at the above referenced site. <br /> Due to the significant contamination present and the close proximity of off site <br /> private and public domestic water supply wells, the San Joaquin Local Health District <br /> requires the following actions be taken: <br /> 1. USA Petroleum shall immediately identify and sample all domestic <br /> water supply wells within 1/4 mile of this facility. <br /> 2. A rush turnaround shall be ordered on those samples identified by the <br /> San Joaquin Local Health District. <br /> 3. USA Petroleum shall submit an unauthroized release report form within <br /> 5 days (see enclosed). <br /> 4. USA Petroleum shall submit a proposal for determining the full extent <br /> of contamination to this office and the Central Valley Regional Water <br /> Quality Control Board within 30 days. <br /> 5. If contamination is found to be present in any of the off site wells <br /> sampled, additional sampling shall be instituted as required by the <br /> San Joaquin Local Health District and the Regional Water Quality Control <br /> Board. <br /> 6.
