2.0 RESPONSIBILITIES <br /> Project Manager Clay Reavis Telephone: 713-466-0980 <br /> The Project Manager is responsible for all on site activities, including having all appropriate plans <br /> prepared and distnbuting the plans to PGE field personnel and to authorize representatives of each <br /> subcontractor employed to perform work on site The Project Manager is also responsible for <br /> ensuring that the provisions of the Site Safety Pian are carried out This includes ensuring that all <br /> PGE field personnel have received adequate training,that an adequate supply of safety and <br /> emergency equipment is available,and that the required safety reports are submitted to the regional <br /> Health and Safety Officer <br /> Site Safety Officer Roy Austin Telephone. 510-732-0143 <br /> Site Safety Officer Duties The Site Safety Officer(SSO)is responsible for implementing the <br /> site safety plan This includes insuring that field personnel have read and signed the Safety Pian, <br /> and that a Tailgate Safety Meeting(TSM)is conducted on the site each day prior to initiating field <br /> activities A sign in sheet will be used to record attendance as well as topics covered for each <br /> TSM In addition, the SSO will be responsible for auditing the site operations for compliance with <br /> the Site Safety Plan and any federal,state or local requirements, The SSO will take air monitoring <br /> readings throughout the work day and be responsible for the site Personnel Protective Equipment <br /> (PPE) level The SSO is required to be on site at all times In the SSO's absence,a designated <br /> alternate wilt assume the duties and responst bi I ittes <br /> 3.0 SITE INFORMATION <br /> ' Source/Age of information Recent <br /> History of Contamination a Tanker truck containing 500 gallons of fuel spilled on the area <br /> Contamination Gasoline, Diesel,Acetone <br /> Work Description Geoprobe <br /> Incidental/Site Description Spill { } Fire ( ) HW Site ( ) Industrial ( ) Facility( ) <br /> Drilling( ) Excavation (X)Other( ), Explain <br /> Physical Description of Facility (Map indicating Exits is attached) <br /> ' Operational description of facility Housed compressors <br /> Site Status Active( ) Inactive(X) <br /> Have Nearby People Been Evacuated9 Yes ( ) No(X) <br /> ' Evacuation Distance 400' <br /> Regulatory Agencies On Site San Joaquin County Health Services <br /> 2 of 6 <br />
