skulwest <br /> AW A 1 R L I N f:ti c� <br /> January 31, 2020 <br /> SJC Environmental Health Department <br /> Attn: Robert Lopez <br /> 1868 E Hazelton Avenue <br /> Stockton, CA 95205 <br /> RE: SkyWest Airlines Inspection Completed December 5 2019 at 5000 S Airport Way, Stockton CA. <br /> Dear Mr. Lopez, <br /> This letter is written in response to the Hazardous Materials Program Inspection Report received by <br /> SkyWest Airlines on December 31, 2019. <br /> In regards to item number 6, HSC 25505 (a)(2) Failed to complete and/or submit a site map with all <br /> required content,SkyWest has added an additional site map with the required designations to California <br /> Environmental Reporting System (CERS) and the maps are attached to this document, marked as <br /> Attachments 1 and 2. <br /> Concerning item number 8, HSC 25505 (a)(4) Failed to submit a training program or program is not <br /> reasonable or adequate for facility. SkyWest Airlines has added additional items to our existing Storm <br /> Water Training course to include the requirements specified. The course has been uploaded to CERS <br /> and is attached here as Attachment 3. <br /> In reference to item number 11, HSC 25505 (a)(4) Failed to provide or document initial and/or <br /> refresher training to appropriate personnel. SkyWest Airlines has assigned all employees at our facility <br /> at the above mentioned address to take the updated course referenced in item number 8. Local <br /> leadership has completed the course and all others will complete it no later than FEB 7, 2020. <br /> And finally, in regards to item number 12, HSC 25505 (c) Failed to have the business plan readily <br /> available to facility or EHD personnel.
