SAN„JOAQUIN <br />_ _ c ,:-_, i_i N I Y <br />Hazardous Materials Program Inspection Report <br />Facility Name: <br />UNIVERSITY OF THE PACIFIC PHYSICAL <br />Facility Address: <br />1050W BROOKSIDE RD, STOCKTON <br />Date: <br />August 08, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # Remarks <br />11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Documentation showing initial and annual refresher training of the following elements was not available at the time of <br />inspection: <br />Familiarity with the emergency response plans and procedures in the event of a release or threatened release of a <br />hazardous material; (Emergency Response/Contingency Plan familiarization). <br />Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br />agency; (spill reporting). <br />Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment; (spill response). <br />Evacuation plans and procedures, including immediate notice, for the business site; (evacuation training). <br />The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher <br />training. This training shall be documented electronically or by hard copy and shall be made available for a minimum <br />of three years. Immediately provide employee training for appropriate personnel and submit a copy of the training <br />records to the EHD. <br />This is a Class ll violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection, August 8, 2019. The EHD <br />has written this complete report which replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
