PHS/EHD has <br /> prepared the following comments for response. <br /> PHS/EHD issued permit#13207 on July 30, 1997 to drill and install monitoring well, MW9, with the <br /> condition that the maximum length of screened interval approved was 20 feet. PHS/EHD wrote the <br /> condition on the permit. A copy of the permit was mailed to Woodward Drilling, the C-57 licensed well <br /> driller. PHS/EHD forwarded to you and your consultant Steve Carter with GeoStrategies by fax a copy of <br /> the permit and a cover letter indicating again the condition of the permit. PHS/EHD also stated on the <br /> cover letter that vapor extraction wells, screened in the vadose zone must be installed to perform an <br /> accurate vapor extraction pilot test and requested that Unocal consider installing the additional well(s) <br /> during the same mobilization. <br /> The field work associated with the installation of MW9 was scheduled for August 19, 1997 with a grout <br /> inspection time scheduled at 1:00 PM. When PHS/EHD arrived on site for the scheduled inspection, the <br /> traffic box had already been installed in the City of Stockton sidewalk and the grout mixed. As the well <br /> was grouted, PHS/EHD discussed with the GeoStrageties representative on site the soils encountered <br /> during drilling, soil sampling depths and well construction details. PHS/EHD was informed that MW9, was <br /> installed with 30 feet of screened interval. <br /> Please note also that PHS/EHD was informed by MPDS correspondence (not dated) that was received by <br /> PHS/EHD on August 4, 1997, that the site was scheduled for quarterly groundwater sampling on August 4, <br /> 1997. PHS/EHD has requested on numerous occasions that at least 48 hours notice be provided prior to <br /> sampling events. <br /> Again, Unocal has failed to comply with regulatory directives and permitting requirements. <br /> If you have any questions regarding this written notice of violation of PHS/EHD regulatory directives, <br /> please provide them in writing. <br /> Donna Heran, REHS, Director <br /> Environmental Health Division <br /> x-02. <br /> Mary Meayseays, Senior <br /> �S Margaret agono.
