Former Unocal Service Station #0187, 437 East Miner <br /> Avenue,Stockton,California <br /> Mr.Montross: <br /> At the request of Unocal Corporation(Unocal), Gettler-Ryan Inc. (GR)is submitting the enclosed Revocable Permit <br /> Applications for the installation of one groundwater monitoring well and six remedial ozone sparge wells,and utility <br /> conduits for the remedial system in the City of Stockton's right-of-way for the above referenced site. The <br /> installation of the wells and the installation of the utility conduits are being requested under separate permits because <br /> of our understanding that the permit process for the installation of the wells can be completed much sooner than for <br /> the utility conduit permit. That will allow GR to install the wells in a timely manner, then return at a later date and <br /> install the utility conduits and supporting remediation equipment. The installation of the wells is outlined in the <br /> enclosed GR report #240042.11, Work Plan for Additional Subsurface Assessment and Remedial Action, dated <br /> November 27,2001. <br /> Included with the Revocable Permit Application package for the wells are a check #057939 in the amount of <br /> $310.00 (processing fee), a bond in the amount of$3,500.00 (7 wells at $500.00/well), a letter from San Joaquin <br /> County Public Health Services(SJCPHS)requesting the work,and two USPS delivery confirmation forms for work <br /> notification letters to adjacent property owners. A current GR Certificate of Insurance should be on file with the <br /> City. <br /> Included with the Revocable Permit Application package for the utility conduit are a check#058086 in the amount <br /> of$310.00(processing fee)and map(Figure 1)describing the work to be done in the City's right-of-way.
