Miner Avenue SITE CODE: 1181
Stockton, CA
San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD)
has reviewed the "Work Plan For Additional Subsurface Assessment" dated November
12, 1999, prepared by Gettler-Ryan Inc. for the above referenced site.
The work plan is approved with the following comments and conditions. Analysis of the
soil samples collected at 10 and 20 feet below surface grade (bsg) in the deep boring
proposed is not required by PHS/EHD. Analysis of the soil sample collected at 65 feet
bsg is required since contamination at this depth was documented in the soil from NP2.
PHS/EHD recommends the well screen on the shallow well extend to 35 feet bsg since
the well screen for NP-1-22 is from 22 to 37 feet bsg and the well screen for NP-2-22 is
from 22 to 47 feet bsg.
Please submit the PHS/EHD well permit applications (2) and $89 permit fee per
application, encroachment permits, access agreements, and health and safety plan.
Once PHS/EHD has approved the well permits, the work can be scheduled. Please
provide PHS/EHD notification of the scheduled work at least 48 hours prior to its
performance.
If you have any questions contact me at (209) 468-3449.
Donna Heran, RENS, Director
Environmental Health Division
'
Margaret Lagorio, Supervising REHS
Site Mitigation Unit
c: CVRWQCB - Mark List
UNOCAL - Bob Boust
SJCADA - David Irey
Gettler-Ryan Inc. — Stephen Carter
A Division of San Joaquin County Heahh Care Services
