John Yoakum [EH]
From: Munn,Julie <Julie.D.Munn@arcadis-us.com>
Sent: Monday, September 08, 2014 1:41 PM
To: Adrienne Ellsaesser [EH];John Yoakum [EH]
Subject: FW:437 East Miner Ave. (306415) - Permit Applications
Attachments: 306415_437 East Miner-Permit applications.pdf, 306415_437 East Miner-All Blast
Plans.pdf; 306415_437 East Miner Table 1.pdf; 306415 Figure 2.pdf
Johnny/Adrienne-
Attached are the 3 permit applications for the upcoming well destructions associated with the former Chevron site no.
306415 located at 437 E. Miner Ave, in Stockton. All monitoring or remedial wells will be destroyed by detonation
methods and the soil vapor probes will be hand augered or over-drilled depending on the depth (per the email
correspondence below). Permit applications, blast plans for individual wells and a table showing the well construction
and destruction methods are attached.
I am sending this via email with the hope that the permit review process can begin right away. I have requested a check
from my corporate office in the amount of$11,830 ($390 for each permit, plus$130 per well/82 wells). I should have
the check by Wednesday and will fedex the entire package including wet signatures from the driller/LCD and me for
arrival on Thursday. It is our hope to get this work going beginning September 22, 2014. Is this enough time for
approving the permits?
Also, can you tell me if you have a Master File Record Information Form for this site already? I believe you should but
wanted to double check.
Thank you,
Julie D.
