The grout used to <br /> seal the well shall then be topped off and allowed to spill over into the excavation and fill <br /> it for a thickness of one foot to form a 'mushroom cap' over the top of the cut casing. <br /> • If 'nested' wells were installed at the site and need to be destroyed by overdrilling, an <br /> auger larger than the original auger must be used and centered on the deepest casing <br /> (which is off-center in the original borehole). The auger needs to be sized sufficiently to <br /> reach out and remove all well casings, screens, filter packs, and grout sealant installed <br /> during construction. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out of <br /> the excavation and containerized for proper disposal. <br /> • Any wells located offsite require updated access agreements and/or encroachment <br /> permits to be submitted with the EHD Well Destruction Permit Application. <br /> Deadline for submittal of the work plan is July 14, 2013. After the EHD has approved the work <br /> plan you may submit an EHD Well Destruction Permit Application along with the appropriate <br /> fees. Current fees are: $375.00 per permit, (one permit per parcel) plus $125.00 per each well <br /> on the parcel that is to be destroyed.
