12 January 2007 <br /> AGE-NC Project No. 96-0232 <br /> Page 3 of 7 <br /> within a 2,000-foot radius. <br /> AGE: 14 May 1998: AGE recommended in the quarterly report collecting a minimum of four grab <br /> ground water samples to complete the delineation of the dissolved hydrocarbon plume and <br /> to determine the necessity for placement of additional ground water monitoring wells, if <br /> necessary. Upon the request of the PHS-EHD, AGE will provide a site specific work plan <br /> addendum for the collection of grab ground water samples.Additionally,AGE recommended <br /> to conduct a receptor survey to identify any city, domestic, irrigation or monitoring water <br /> wells located within a 2,000-foot radius. <br /> AGE: 13 October 1998: AGE recommended in the quarterly report that PHS-EHD has verbally <br /> indicated that soil and possibly ground water remediation are necessary at the site and that <br /> a Corrective Action Pian (CAP) should be prepared. The CAP should include a minimum <br /> of three remediation feasibility studies (e.g. in-situ air sparging pilot test, soil vapor <br /> extraction,select bio-enumerations for natural attenuation,soil excavation and ground water <br /> pump and treat)to determine appropriate remedial options for the site. Additionally, AGE <br /> recommended a ground water well survey of city, domestic, irrigation or monitoring water <br /> wells located within a 2,000-foot radius should be included in the CAP. <br /> AGE: 24 March 1999: AGE recommended in the quarterly report to prepare a CAP as verbally <br /> indicated as required by the PHS-EHD for ground water remediation at the site. The CAP <br /> should include a minimum of three remediation feasibility studies(e.g.ex-situ pilot tests and <br /> select bio-enumerations for natural attenuation) to determine appropriate remedial options <br /> for the site. Additionally, AGE recommended to plan and implement abatement of the free <br /> product encountered in MW-4. <br /> EHD: 21 April 1999: EHD directed full definition of the vertical and lateral extent of soil and <br /> ground water contamination and perform feasibility studies to evaluate remedial alternatives <br /> for cleanup. After the feasibility study is completed, a CAP is required to be submitted by <br /> O1 August 1999. <br /> AGE: 10 June 1999: AGE recommended in the quarterly report to prepare the CAP as required by <br /> the PHS-EHD for ground water remediation at the site.The CAP should include a minimum <br /> of three remediation feasibility studies (e.g. ex-situ pilot tests and select bio-enumerations <br /> for natural attenuation)to determine appropriate remedial options for the site. <br /> AGE: 21 February 2000:AGE recommended in the quarterly report to prepare the CAP as required <br /> by the PHS-EHD for ground water remediation at the site.The CAP should include evidence <br /> from remediation feasibility studies (e.g. ex-situ pilot tests and select bio-enumerations for <br /> Advanced GeoEnvironmental,Inc. <br />
