PHS/EHD has a <br /> contract with California State Water Resources Control Board to conduct an <br /> underground storage tank program. The above referenced site was placed in the <br /> PHS/EHD Local Oversight Program (LOP) in July 1994. <br /> On August 4, 1998, PHS/EHD met with you and your consultant, Advanced <br /> GeoEnvironmental, Inc., to review the investigation and remedial activities for the above <br /> referenced site. We reviewed site history, the findings from the investigation, and <br /> analytical results from monitoring events. <br /> In April 1994, six USTs were removed from three separate excavations on the above <br /> referenced site; one waste oil UST, one diesel UST, and four unleaded gasoline USTs. <br /> An unauthorized release was reported in the area of the former diesel UST. PHS/EHD <br /> gave a directive to perform additional investigation and any necessary corrective action <br /> to mitigate the release. <br /> A soil and groundwater investigation has been performed. In December 1995, five soil <br /> probe borings were advanced on site. In June of 1996, four monitoring wells (MW) were <br /> installed; three.on site and one off site. Groundwater contamination was documented. <br /> Free phase diesel has been observed in the MW-4 on site. Gasoline and diesel have <br /> been documented off site in MW-1. The full extent of groundwater contamination has <br /> not been defined in the direction of MW-1. <br /> A well survey was conducted in June of 1997.-Three wells were identified in the survey. <br /> All three wells were constructed with a 50-foot sanitary seal and gravel packed. One <br /> well is within 350 feet southwest of the site at a private residence. <br /> Boring logs and soil sample results indicate contamination exists as deep as 25 feet <br /> below ground surface. You are directed to fully define the vertical and lateral extent of <br /> soil and groundwater contamination and perform feasibility studies to evaluate remedial <br /> alternatives for cleanup. After the feasibility study is completed, you are required to <br /> submit a Corrective Action Plan by August 1, 1999. <br /> A Division of San Joaquin Counry Heafch Care Services <br />
