SAN JOAQUIN <br />IIIIVIISJIIIIIVIIICIIIIVCIIIII LJCIJCIIIIIIVIll <br />Hazardous Materials Program Inspection Report <br />Facility Name: <br />UNIVERSITY OF THE PACIFIC <br />Facility Address: <br />3601 PACIFIC AVE, STOCKTON <br />Date: <br />July 29, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # Remarks <br />10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br />During the inspection approximately 85 gallons of Sanacor 2621, At least 2,000 gallons of diesel fuel, 1,000 pounds of <br />calcium chloride, greater than 55 gallons of transformer oil, greater than 55 gallons of hydraulic oil, and containers of <br />calcium metal, lithium metal and sodium metal were found on site that have not been reported and the updated <br />information has not been submitted. Within 30 days of one the following events, business shall electronically update <br />the information submitted to the California Environmental Reporting System (CERS): <br />- A 100 percent or more increase in the quantity of a previously disclosed material. <br />- Any handling of a previously undisclosed hazardous materials <br />- A change of business address, business ownership, or business name. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Documentation showing initial and annual refresher training of the following elements was not available at the time of <br />inspection: <br />Familiarity with the emergency response plans and procedures in the event of a release or threatened release of a <br />hazardous material; (Emergency Response/Contingency Plan familiarization). <br />Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br />agency; (spill reporting). <br />Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment; (spill response). <br />Evacuation plans and procedures, including immediate notice, for the business site; (evacuation training). <br />The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher <br />training. This training shall be documented electronically or by hard copy and shall be made available for a minimum <br />of three years. Immediately provide employee training for appropriate personnel and submit a copy of the training <br />records to the EHD. <br />This is a Class ll violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection, July 29, 2019.
