The replicate receives a different identification number which will be assigned by the QA/QC officer. At a minimum, the following items will be addressed: o A description of the laboratory organization structure, its key personnel, their specific duties, and credentials. o The objectives of the laboratory QA/QC plan. o Sample custody and documentation procedures. o Instrument calibration procedures and frequency. o Instrument preventative maintenance schedule. o Analytical procedures. ' o Data reduction, validation and reporting- procedures. ' o Performance audits, system audits, and internal quality control checks. ' o Assessment procedures for precision and accuracy. o Corrective action procedures. ' o Quality assurance reporting to management. OHM uses a laboratory certified under DHS's Hazardous Waste ' Laboratory Certification Program. 262
