Weber Avenue, Third floor ENVIRONMENT HEALTH <br /> Stockton, California 95202 PERMIT/SERVICES <br /> Subject: Well/Receptor Survey <br /> MEL BOKIDES PETROLEUM - MOSSDALE <br /> 444 Mossdale Road, Lathrop, California <br /> Dear Ms. Oz: <br /> At the request of Mr. Nick Bokides of Mel Bokides Petroleum, Inc. and as requested by the San <br /> Joaquin County Public Health Services - Environmental Health Division (FHS-EHD), Advanced <br /> GeoEnvironmental, Inc. (AGE) has prepared the enclosed report of wells located within 2,000 feet <br /> of the site located at 444 Mossdale Road, Lathrop, California. <br /> AGE has prepared a water well survey for the site. A well survey was performed to identify city, <br /> domestic, irrigation or monitoring water wells located within a 2,000-foot radius of the subject site. <br /> The area was visually inspected for wells. The scope of the work included the reconnaissance for <br /> visible potential receptors(address and location), a review of files and records available from the San <br /> Joaquin County Public Health Service - Environmental Health Division (PHS-EHD), Department <br /> of Water Resources (DWR) and San Joaquin County Public Works Department (SJCOPWD). <br /> Furthermore, a search of the on-site or nearby septic systems that could potentially serve as a <br /> pathway to surface ground water. The location of the site and the immediately surrounding area are <br /> illustrated on Figure 1. Site structures are depicted on Figure 2. <br /> RESULTS <br /> Based on the visual observations and data collected from the above mentioned regulatory agencies <br /> the following wells were located in the search area. <br /> • One domestic water wells service the site. The well is located 100 feet north west of the <br /> known extent of MTBE and serves as the main water source for the site. A well logs for the <br /> 4005 North Wilson Way, Stockton.
