NARRATIVE <br /> El Camino Tire <br /> Tire Program ID number. Mr. McClellon told Mr. Montijo to obtain an EPA number for <br /> the facility. <br /> Mr. Backus gave Mr. Montijo several documents which explain the proper handling and <br /> disposal of hazardous waste. Mr. McClellon informed Mr. Montijo that the <br /> Environmental Health Department (EHD) provides free training for hazardous waste <br /> generators. Mr. McClellon advised Mr. Montijo to call Kasey Foley, supervisor for the <br /> hazardous waste generator program,to get further information and guidance. <br /> Mr. McClellon and Mr. Backus wrote an EHD hazardous waste generator program <br /> inspection report and a waste tire program inspection report for the facility. Mr. <br /> McClellon and Mr. Backus discussed the inspection reports with Mr Montijo. Mr. <br /> Montijo signed the reports and received copies of all pages. <br /> Mr. McClellon and Mr. Backus took samples D-1, D-2 and D-3 back to the EHD and <br /> stored them in the third floor refrigerator in the Emergency Response room. <br /> May 30, 2006: Laboratory results for the ignitability test were received by Mr. Backus <br /> for samples D-1, D-2 and D-3. GeoAnalytical Laboratories Inc. (1405 Kansas Ave, <br /> Modesto, CA 95351) performed the tests. Samples D-1, D-2 and D-3 had flashpoints of <br /> 90 °F, 75 °F and <70 °F, respectively. All three samples have the hazardous waste <br /> characteristic of ignitability since they have a flashpoint of less than 140 °F. The <br /> characteristic of ignitability is defined by Title 22 California Code of Regulations <br /> §66261.21. <br /> June 15, 2006: Mr. Backus met Mr. Montijo at the EHD office at Mr. Montijo's request. <br /> Mr. Montijo wanted help filling out tire and hazardous waste forms. Mr. Montijo had <br /> filled out California Department of Toxic Substances Control (DTSC) form 1358 to apply <br /> for a California ID number for hazardous waste generators. He also filled out California <br /> Integrated Waste Management Board (CIWMB) forms 60 and 61 to register his trucks for <br /> waste tire hauling. Mr. Montijo asked Mr. Backus to process the forms. Mr. Backus <br /> replied that the hazardous waste and tire forms must be sent directly to DTSC and <br /> CIWMB, respectively. Mr. Montijo agreed to mail the forms. <br /> June 26, 2006: Laboratory results for the bioassay test were received by Mr. Backus for <br /> samples D-1, D-2 and D-3. Samples D-1 and D-2 have the hazardous waste <br /> characteristic of toxicity since they have an acute aquatic 96-hour LC50 of<500 mg/L. <br /> The characteristic of toxicity is defined by Title 22 California Code of Regulations <br /> §66261.24. <br /> Garrett Backus, REHS <br /> Environmental Health Department O <br /> Page 3 of 3 <br />
