September 5, 1991 <br /> Page 2 of 2 <br /> The information on these labels will correspond to the Chain-of-Custody <br /> Record which shows the identification of individual samples and the contents <br /> of the shipping container The original record will accompany the shipment, <br /> I <br /> and a copy will be retained by MARK <br /> When transferring samples, the individual relinquishing and receiving <br /> will sign, date, and note the time on the record This record documents <br /> sample custody transfer <br /> Samples are packaged properly for shipment and dispatched to the <br /> appropriate laboratory for analysis, with a separate Chain-of-Custody Record <br /> accompanying each shipment Shipping containers are padlocked or sealed with <br /> ® custody seals for shipment to the laboratory <br /> A designated sample custodian accepts custody of the shipped samples and <br /> verifies that the information on the Sample Identification number matches <br /> that on the Chain-of-Custody Records Pertinent information as to shipment, <br /> pickup, and courier is entered in the "Remarks" section Once the laboratory <br /> custodian receives the samples, laboratory personnel are responsible for the <br /> care and custody of the samples <br /> 7 2 Laboratory Procedures <br /> Laboratory chain-of-custody procedures are specific to each commercial <br /> laboratory <br /> 7-2 <br />
