(AGE) for the above referenced site. EHD is in <br /> agreement with the recommendations of AGE contained in the revised addendum. <br /> EHD still has not received a complete well survey report of domestic, municipal, <br /> industrial and other wells within a 2,000-foot radius of this site, as requested in letters <br /> dated 26 April 2000 and 06 June 2002. Contact the Department of Water Resources, <br /> municipal water suppliers, and nearby industries for well information. Perform a <br /> physical search of the area to identify all wells. Show the location of all wells on a map, <br /> relative to the location of this site. Submit any known construction details of the wells, <br /> and identify all sources of information in the well survey report. Submit this report by <br /> April 14, 2003. <br /> A site conceptual model (SCM) should be prepared for this site. This is a written <br /> description supported by maps, geological cross-sections, tables, and diagrams that <br /> provide information about the site. The SCM may include the following: <br /> I. Local and regional plan view maps showing location of sources, extent of <br /> contamination (i.e., an interpretive drawing - not merely a plot of laboratory <br /> results), direction and rate of groundwater flow, and locations of receptors. <br /> "Receptors" include,but are not limited to, all supply wells within 2000 feet of <br /> the source area-, <br /> 2. Cross-section maps showing subsurface geologic features, depth of groundwater, <br /> man-made conduits, monitoring well construction, and an interpretive drawing of <br /> the vertical extent of soil contamination-, <br />
