.1r <br /> Advanced <br /> GeoEnvlronm <br /> ' ental nc • <br /> �� <br /> �% NPR 2a <br /> 19 April 2006 t o,jwf I <br /> AGE-NC Project No. 98-0534 F� ��y14 {�H,, f4 t NL <br /> EN � tjOIAES-f <br /> UE? <br /> JAE AL <br /> Ms. Vicky McCartney <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue <br /> Stockton, California 95202 <br /> Subject: Monitoring Well Destruction Work Plan Addendum <br /> Former MEL BOKIDES PETROLEUM <br /> 2191 Navy Drive,Stockton,California <br /> Dear Ms. Vicky McCartney: <br /> In accordance with your directive letter dated 31 March 2006,Advanced GeoEnvironmental,Inc.(AGE) <br /> has prepared this work plan addendum regarding the proper destruction of monitoring wells at 2191 <br /> Navy Drive,Stockton,California.The above-reference letter is included as Appendix A;location of the <br /> site is illustrated in Figure l; site structures and monitoring well locations are depicted on Figure 2. <br /> In order to ensure that ground water monitoring wells do not provide a vertical conduit for <br /> contaminants, AGE proposes to properly destroy them in accordance with San Joaquin County <br /> Environmental Health Department (EHD)Standards For Well Construction and Destruction. <br /> PROCEDURES <br /> As required by EHD permitting, a health and safety plan will be prepared in accordance with <br /> Occupational Safety and Health Guidance Manual for Hazardous Waste Site Activities (National <br /> Institute for Occupational Safety and Health Administration, U.S. Coast Guard and U.S. <br /> Environmental Protection Agency, 1985).Prior to drilling activities,each proposed drilling location <br /> will be clearly marked and an utility clearance obtained through Underground Service Alert. <br /> Based on the EHD "Standards"the wells will be sounded for total depth and checked for integrity, <br /> any obstruction found in the well will be removed. Well boxes and caps will be removed to expose <br /> the casing. As required by the Standards, wells showing no history of contaminant impact will be <br /> destroyed by pressure grouting; impacted wells (even if currently non-detect) will be destroyed by <br /> complete over-drilling. <br /> Due to historical analytical results from well installation, groundwater monitoring, and vapor <br /> sampling activities, only MW-1 is eligible for destruction by pressure grouting; the AGE-prepared <br /> Revised Closure Summary Report dated 08 August 2005 and prepared for the site includes all <br /> relevant background and analytical data. Although groundwater monitoring samples from deep- <br /> screened MW-9 have been historically non-detect for constituents of concern,the location of the well <br /> 837 Shaw Road..
