Main Street <br /> Attachments: 2011 Well & Boring Permit Application.pdf; MFR.pdf <br /> Good afternoon Jim, <br /> A copy of the County's Well and Boring Permit Application, Master File Record (MFR)form, and Licensed <br /> Contractors Declaration (LCD)are appended for you to review and use. As I mentioned during our phone <br /> conversation today, monitoring wells MW-1 through MW-3 have already been installed at the site and soil borings <br /> SB-1 through SB-3 have already been advanced at this site. Please resubmit the permit application for one on- <br /> site soil boring (SB-4) and two temporary soil vapor probe borings. The fee is$125.00 for the borings and <br /> $375.00 for the permit/three-hour inspection fee for a total of$500.00 for the on-site fieldwork. You do not have <br /> to resubmit the permit application for installing the two off-site monitoring wells. The fee for the two wells is <br /> $125.00 times 2 plus a permit/inspection fee of$375.00, making the total $625.00 for the two off-site wells. You <br /> must submit a copy of the Encroachment Permit when submitting the permit and fees since I can not issue the <br /> permits without this document. Please do not hesitate to contact me if you have any questions concerning this <br /> site. <br /> I am not available October 10th through October 17th, October 21 st, and October 24-25, 2011. Once you have <br /> scheduled the work with the drilling company, please let me know so that I can arrange to be at the site for the <br /> inspection.
