how <br /> STANDARD OPERATING PROCEDURES FOR <br /> GROUNDWATER MONITORING WELL DESTRUCTION <br /> 1.0 OBJECTIVE <br /> Destroy groundwater monitoring wells no longer in service. <br /> 2.0 BACKGROUND <br /> Following case closure, monitoring wells that are no longer needed will be properly destroyed, as <br /> required by local and state laws. Destruction of unnecessary monitoring wells prevents the potential <br /> threat of a vertical path for contaminant migration. <br /> 3.0 PERSONNEL REQUIRED AND RESPONSIBILITIES <br /> Staff Geologist: An experienced staff geologist (SG) ensures that the monitoring wells are properly <br /> destroyed/abandoned. The SG is responsible for complying with the stated procedures regarding well <br /> destruction and documentation. <br /> Drilling Technicians: Drilling technicians from a California licensed drilling company perform the <br /> monitoring well destruction. <br /> 4.0 EQUIPMENT REQUIRED <br /> • Drill Rig <br /> • Level D Safety Equipment (or higher as required) <br /> • Well Completion Detail of Existing Well <br /> • Boring Log form <br /> Disposable gloves <br /> 5.0 PROCEDURE <br /> • GHD obtains required permits prior to destroying the monitoring well(s). A Site-Specific Health <br /> and Safety Plan detailing site hazards, site safety, and control has been prepared prior to any <br /> field work. Underground Services Alert (USA) is notified of the planned work at least 48 hours <br /> prior to drilling (notification to USA is important to determine if utilities have been placed <br /> adjacent to the well subsequent to the installation of the monitoring well). <br /> • A photo-ionization detector(PID)will be used during the destruction activities to screen for the <br /> presence of volatile organic compounds (VOCs). <br /> • A boring log is completed to document the details of the well destruction procedure, and a <br /> California Department of Water Resources (DWR) Well Completion Form No. 188 is completed <br /> and forwarded to DWR for a permanent record that the well has been destroyed. <br /> 03009-10016-37140 1 of 2 October 2013 <br />
