March Lane <br /> Stockton, CA 95210 <br /> On November 7, 1991, San Joaquin County Public Health Services, Environmental Health Division <br /> (PHS/EHD) received a "Site Mitigation Acknowledgment/Request for Services' form signed by David <br /> Jermstad of Youngdahl & Associates, Inc. dated November 5, 1991 indicating that Youngdahl & <br /> Associates, Inc., was to be named as the 'responsible party' for billing purposes. <br /> Site specific charges were accrued as the workplan was reviewed and a file was started for this site. <br /> On November 18, 1991 PHS/EHD received correspondence from Youngdahl & Associates, Inc., <br /> requesting that "the County hold our Application'. <br /> PHS/EHD will keep the file open on this investigation until laboratory sheets from 11 October 1991 are <br /> received and reviewed.
