The <br /> remaining employees on site had not been trained by the designated operator. The designated operator shall train <br /> facility employees for which he or she is responsible in the proper operation and maintenance of the UST system <br /> once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site. The manager was <br /> trained by the designated operator at the time of inspection. <br /> This is a Class II violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> A violation was cited in the May 16, 2016 inspection report stating primary containment leaks were observed under <br /> dispensers 7/8 and 9/10. No permit has been obtained for primary piping repairs. No maintenance records were <br /> found on site for repairs at these dispensers. No follow up was submitted addressing these repairs. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a Class II violation. <br /> 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br /> Water was found in the fill and turbine sumps on site. At the time of arrival on site, the technicians had already <br /> removed approximately 20 gallons of water from the sumps. Additional water was found in the vent sump and the <br /> diesel fill sump. More than 18 inches of water was removed from the diesel fill sump. Secondary containment shall <br /> be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The <br /> service technician removed the liquid from the sumps and returned the sensors to their proper location. Immediately <br /> contact a properly licensed, trained, and certified contractor to address the water intrusion into the vent, fill, and <br /> turbine sumps under permit and inspection of the EHD. <br /> This is a Class II violation. <br /> 326 HSC 25299(a)(9)Tampered with or disabled leak detection devices or alarms. <br /> The diesel fill sump sensor was raised above the water level in the sump; more than 18 inches of water was found in <br /> the diesel fill sump at the time of inspection. Monitoring equipment shall be maintained to be able to detect a leak at <br /> the earliest possible opportunity. The water was removed by the service technician and the sensor was placed in <br /> the proper location at the time of inspection. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Page 6 of 7 <br />
