4.0 INVESTIGATIVE WORK
4.1 Workplan Submission
On March 15,2005 Mr. Patel retained this office to prepare and implement a workplan to
complete the limited site assessment at the St. Francis Motel site. The workplan was approved by
PHSEHD in a letter dated June 14, 2005.
Additional background work was conducted by this office during September 2005 to determine
the location of the septic tank and lines leading into and out of the system. On November 18,
2005 PHSEHD directed Mr. Patel to implement the approved workplan. Mr. Patel was out of
the country until late January,2006 and no work was conducted during his absence. On February
15, 2006 a hearing was held for the Board of Housing Appeals of the County of San Joaquin
which issued a direction letter to Mr. Patel to take necessary action to stay in compliance with the
PHSEHD directives. An amended workplan was submitted by this office for Mr. Patel to
PHSEHD on February 14, 2006. The workplan was verbally approved by PHSEHD and work
started during March,2006.
4.2 Septic Tank Sampling
On March 29,2006 the St. Francis Motel septic tank was sampled by GeoAnalytical Laboratories
Inc. personnel under direction of Muir in the presence of PHSEHD staff.
