• Fire extinguisher(foam, dry chemical, or carbon dioxide) <br /> • Flashlight <br /> 6.0 TRAINING <br /> All personnel who may be exposed to onsite contaminants must provide documentation <br /> of the following: <br /> • Current training that meets the requirements of 29 CFR 1910.120 and 8 CCR 5192,to <br /> include: <br /> 0 40 hours of classroom instruction/hands-on training <br /> o Three days of field experience under the supervision of an experienced supervisor <br /> o Eight hours of annual classroom refresher training, as appropriate <br /> • Eight hours of supervisory training as specified in 29 CFR 1910.120 if a person is a <br /> designated supervisor. <br /> Project-specific training and information will be provided either before traveling to the <br /> site or at the site before entry into potentially contaminated areas onsite. The information <br /> and training will be documented, and will include the following: <br /> • The contents of the HASP <br /> • A discussion of the site-specific health and safety hazards,protective measures, and <br /> work practices. <br /> 7.0 MEDICAL SURVEILLANCE <br /> Prior to being assigned to a hazardous or a potentially hazardous activity involving <br /> exposure to toxic materials, employees must receive a baseline physical exam. The <br /> contents of the physical exam are to be determined by the employer's medical consultant. <br /> The baseline physical exam should categorize employees as fit-for-duty and able to wear <br /> respiratory protection. <br /> In addition to the baseline physical, employees must have a periodic physical exam every <br /> 12 months.
