(Stantec) submits this addendum to Stantec's May 4, 2012 Well Destruction Work
Plan (Work Plan), which was approved by the California Regional Water Quality Control Board
(RWQCB) in their letter dated May 18, 2012.
Tesoro Companies Inc. (Tesoro) was contacted by Ms. Rita L. Koehnen, Environmental
Coordinator at the Port of Stockton regarding fast-track construction being conducted north of the
STTC site that will require the destruction of monitoring well OW-1A. This issue was briefly
discussed at the May 8,,2012 meeting with Tesoro and the RWQCB indicated a brief addendum to
the May 4, 2012 Work Plan should be submitted. Monitoring well OW-1A is a located
approximately 100 feet north of West Washington Street and northeast of the intersection with Port
Road #23 (Attachment 1, Site Plan). It is a shallow-zone well situated cross-gradient from the Site,
installed in 2002 to 20 feet below grade (Attachment 2, Boring Log OW-1A), and is typically non-
detect for all analytes.
The well will be destroyed in accordance with the procedures outlined in the approved May 4, 2012
Work Plan or by pressure grouting with the approval of San Joaquin County.
Due to the fast-track construction schedule, STTC and the Port of Stockton respectfully requests an
expedited review and approval of this Addendum so that the well can be destroyed next week,
pending receipt of the well destruction permit from San Joaquin County.
