A copy of the permit is included in <br /> Attachment A. <br /> Field Date(s): March 26,2008. <br /> CRA Staff: Carmen Rodriguez, Staff Geologist. <br /> Purpose: Well MW-27 was destroyed to facilitate offsite construction. <br /> Destruction Method: Well MW-27 was destroyed by 1)removing the wellhead vault box; 2)excavating <br /> an area measuring approximately 2 feet in diameter and approximately 3 feet below grade (fbg) around <br /> the casing; 3) filling the well casing with neat cement from the bottom up; 4) applying a pressure seal to <br /> the top of the well casing and pressurizing the well casing for 5 to 10 minutes; 5) cutting the casing off at <br /> approximately 2.5 fbg; and 6) additional grout in the casing and allowing the grout to spill over the top of <br /> the casing and fill the hole to grade. <br /> Waste Disposal: No waste material was generated during the well destruction. <br /> Equal <br /> Employment <br /> Opportunity Employer <br /> Worldwide Engineering, Environmental, Construction,and IT Servmes <br />
