Levine-Fricke-Recon • • DRAFT* z <br /> 2.4 Evaluate Abandonment Procedures for Former LVMD Wells <br /> The DQOs for this task are to assess the adequacy of the abandonment of wells <br /> LVMD #1, LVMD #2, LVMD #4, and LVMD #6 and evaluate whether these former <br /> wells could act as potential conduits for groundwater containing Hazardous Substances <br /> to infiltrate to deeper groundwater zones. At the time that the original scope of work <br /> was developed, as described in Section 3.3.5 of the RI/FS Work Plan, available data <br /> indicated that well LVMD #4 was active. A review of documents obtained from SJC <br /> DPW during this task indicated that well LVMD#4 was abandoned concurrent with <br /> abandonment of wells LVMD #1, LVMD #2, and LVMD #6, and the scope of work <br /> for this task has been modified accordingly. <br /> This task was accomplished by reviewing the following data: <br /> 1 <br /> • Original well construction and well logs for wells LVMD #1, LVMD #2, LVMD <br /> #4, and LVMD #6 described in the Well Data forms and Water Well Drillers <br /> Reports, presented in Appendix A. <br /> Video survey logs for wells LVMD #1, LVMD #2, and LVMD #6 provided in the <br /> Technical Appendices for the Phase II Site Assessment Report (KJC 1991a) and <br /> included as Appendix D of this report. <br /> • Well repair records for wells LVMD #1 and LVMD #6 from record drawings <br /> prepared by SJC DPW (SJC DPW 1994), presented in Appendix E. <br /> • Abandonment procedures for wells LVMD #1, LVMD #2, LVMD#4, and <br /> LVMD #6 described in "Plans and Specifications (Including Special Provisions, <br /> Notice to Contractors, Proposal and Contract) for the Lincoln Village Well <br /> Destruction," SJC DPW, dated May 1, 1992 (including Addendum No. 1, May 29, <br /> 1992), presented in Appendix F, and record drawings for these wells, presented in <br /> Appendix E. <br /> The chronology and details of the original well construction, well repair (if <br /> applicable), and abandonment of wells LVMD #1, LVMD #2, LVMD #4, and <br /> LVMD #6 are presented in Figures 9 through 12, respectively. <br /> Results of the well abandonment assessment are described in Section 3.4 of this report. <br /> 3.0 DISCUSSION OF RESULTS <br /> 3.1.
