Nakano C1l /I D <br /> Leedshill-Herkenhoff, Inc. <br /> 303 Second St. , Suite 88ON <br /> San Francisco, CA 94107 <br /> RE: Lincoln Village Center; Pacific Avenue and Benjamin Holt , <br /> Stockton, CA <br /> In the event aspects of your discussion with my staff were unclear, I would <br /> like to provide clarification. <br /> Regulations pertaining to the discharge of any water, driller' s mud, or <br /> soils associated witn construction and sampling of soil borings or monitor- <br /> ing wells into sanitary sewer or storm drain is prohibited unless specific <br /> approval is granted by California Regional Water Quality Control Board, <br /> the San Joaquin Local Health District , and San Joaquin County Public <br /> Works. Soil cuttings, driller' s mud and water from construction and <br /> sampling shall be containerized properly for transportation offsite to <br /> an appropriately licensed disposal site with prior approval by all <br /> regulatory agencies (CRWQCB, APCD, DOHS, SJLHD) . <br /> Department policy has been established concerning the owner' s release <br /> of analytical data. It is a requirement of the San Joaquin County Well <br /> Ordinance that all environment subsurface assessment drilling/boring <br /> permits be issued only to C-57 licensed contractors (S.J. Co. Ordinance <br /> Code Section 5-4103) . The only exception to the licensure requirement is a <br /> written waiver from the classification issued by the California State <br /> Contractor' s Licensure Board and concurrence with the San Joaquin Local <br /> Health District . <br /> Furthermore , pursuant to the San Joaquin Local Health District fee schedule <br /> Resolution No. 85-58, a $35 .00 consultation fee shall be submitted along <br /> with the completed general Environmental Health application form for review <br /> of the above referenced environmental assessment/remediation activity. <br /> A copy of your completed application is enclosed. <br /> Each additional hour over the initial one hour of time spent in review <br /> activity will be billed at the rate of $35.00 per hour. The additional <br /> amount due will be billed to the applicant identified on the general <br /> Environmental Health application form at the completion of requested <br /> activity , or monthly , if longer that 30 days is required for completion. <br />
