To receive approval for site closure, a <br /> final remediation plan (FRP) shall be submitted in report form to <br /> PHS/EHD, complete with stockpiled soil disposition, transport <br /> documents, and documentation of satisfactory backfill. A request <br /> for closure shall be included in this report at time of <br /> submittal. <br /> Normally, results from any unwitnessed remediation activity at a <br /> site under PHS/EHD regulatory oversight will not be accepted as <br /> evidence towards closure. PHS/EHD feels that this one time <br /> exception does not create a significant adverse environmental <br /> impact.
