To request a waiver, complete the enclosed "Permit Waiver Request" form <br /> and return with any additional information requested below. <br /> 2. Submit your Federal Income Tax Returns for 1994, 1995 & 1996, Form 1040 <br /> and Form 1120 (corporate). <br /> 3. Who owned the tanks and is paying for the cleanup? Nelson's Ready Mix or <br /> Russell Nelson?
