CA 95670 ATTN: CLINTON HARMS TEL : (916) 861-0400 I and subject to the following, PERMISSION IS HEREBY GRANTED to: PERMITTEE Install one groundwater monitoring well approximately 30' from the center line of the State Route 99 Frontage Road/On Ramp and Hammer Lane in Stockton in San Joaquin County as shown in the attached site plan. The monitoring well will have a 2"diameter and a maximum depth of approximately 65' below ground surface. Installation of monitoring well shall in no way impede public traffic. Abandonment and destruction of water well within the State Highway right of way must be handled with requirements established by statute and by agreement with the Department of Water Resources. Sections 13700 to 13806 of the California Water Code. Permittee should furnish a copy of collected survey data to Caltrans without charge(if requested). In accordance with General Provision No. 6, TWO WORKING DAYS PRIOR to the start of this work, permittee shall notify the State representative MARCEL DE LA CRUZ, P.O. Box 2048 Stockton, CA 95201, (209) 948-3656. All work under this permit must meet with the approval of the State representative. Failure on the permittee's part to comply with any provision will be cause for revocation of this permit.
