Box 1257 San Ramon, CA 94583 Attn: Sergio Morescalchi 925-275-3807 and subject to the following, PERMISSION IS HEREBY GRANTED to: PERMITTEE Install two (2) groundwater monitoring wells, maintain, and sample wells along State right of way at Hammer Lane northwest ramp and North Frontage Road (Highway 99) in the City of Stockton in San Joaquin County as shown on the attachment. Upon the completion of groundwater monitoring, the well will be abandoned by pressure grouting with near cement to the ground surface. The well drilling and installation may be conducted at night to reduce the impact of traffic. A copy of this permit shall be placed in the vehicles involved in doing this work. Permittee shall notify the State representative in the area before doing any work under this permit. This permit shall be renewed before expiration date. NO LANE SHALL BE CLOSED AT ANY TIME IN DOING THIS WORK. A pre-job meeting with the assigned Department Representative, Joe Valdez, 948-3656, is required prior to start of any work under this permit! Failure to do so may result in permit revocation with no prejudice. THIS PERMIT IS NOT A PROPERTY RIGHT AND DOES NOT TRANSFER WITH THE PROPERTY TO A NEW OWNER.
