Work Plan For Well <br /> ARCADES Destructions <br /> Chevron Site ID 309075 <br /> January 2,2013 <br /> 3.0 PROPOSED SCOPE OF WORK <br /> The monitoring well network consists of seven groundwater monitoring wells (MW-1, MW-2, <br /> MW-3, MW-4, MW-5, MW-6, and MW-6D) as shown on Figure 2. Well construction details are <br /> included in Table 1. The proposed methodology for the destruction of site related monitoring <br /> wells are discussed below. . <br /> 3.1 SITE SPECIFIC HASP <br /> I <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous Waste <br /> Operations and Emergency Response" guidelines (29 Code of Federal Regulations Section <br /> 1910.120), and by California Occupational Health and Safety Administration (Cal-OSHA) <br /> "Hazardous Waste Operations and Emergency Response" guidelines (California Code of <br /> Regulations Title 8, Section 5192), ARCADIS will prepare a health and safety plan (HASP) prior <br /> to commencement of fieldwork. Field staff and contractors will review the HASP before <br /> beginning field operations at the site. <br /> 3.2 PERMITTING <br /> Applicable permits will be obtained from the City of Stockton and SJCEHD as necessary, prior <br /> to commencing field activities. <br /> 3.3 UNDERGROUND UTILITY LOCATING <br /> Underground Service Alert (USA) will be notified a minimum of 48 hours before beginning any <br /> evasive operations (i.e. drilling, etc) to identify any public utilities in the vicinity of the proposed <br /> 1. <br /> well destruction locations. In conjunction with the USA, private utility locate and ground <br /> penetrating radar (GPR) activities'will be completed to further evaluate underground utilities. <br /> 3.4 WELL DESTRUCTION ACTIVITIES <br /> 3.4.1 Complete Over Drill <br /> Monitoring wells MW-1, MW-2, MW-3, MW-4, MW-5, and MW-6 will be destroyed by removing <br /> the well box and over drilling with 10-inch outside diameter (OD) hollow stem augers (HSA) to <br /> total completion depth to remove all material (e.g. casing, filter pack, annular seal, etc.) <br /> associated with the well. The boring will then be grouted from the bottom of the borehole to <br /> within 3 feet of ground surface using a tremie pipe and cement-bentonite grout.
