No downgradient well exists to further define the extent of groundwater <br /> contamination. A replacement well is required for MW 9 and a workplan that addresses <br /> its new location along with the next phase of site investigation is now required and shall <br /> be submitted to PHS/EHD within forty five (45) days of this letter. Include in this <br /> workplan, proposals to define all lateral extents of the groundwater plume as well as <br /> steps to bring MW 6 into an active status or locations for its replacement and destruction <br /> of the inactive well. <br /> Continue to sample and report to PHS/EHD and the Central Valley Regional Quality <br /> Control Board (CVRWQCB) groundwater information on a quarterly basis. Reports are <br /> due by the 15th of the month following the quarters end. The quarterly report originally <br /> due by October 15, 1992 may be submitted as late as November 30, 1992 due to recent <br /> changes in the site's groundwater investigation. Sampling of groundwater monitoring <br /> wells shall resume on a regular quarterly basis and the fourth quarter report for 1992 <br /> shall be submitted to PHS/EHD and CVRWQCB by January 15, 1993. <br /> PHS/EHD considers witnessing of sampling events an important aspect of regulatory <br /> oversight and requests that appointments be confirmed with Mr. Infurna 48 hours in <br /> advance. <br /> A Division of Sun Joaquin foum Health fare S ces <br />
