(EAI) to collect a ground water sample (on behalf of Montgomery <br /> Ward) from beneath the west side of Pacific Avenue, south of Robinson Street on November <br /> 11, 1996. This location has been cleared by Underground Service Alert. The ground water <br /> sample will be collected using the Hydropunch technique, a one time sampling event, i.e., no <br /> permanent well will be installed. The ground water sampling is being required by the an <br /> Joaquin County Public Health Services (County). The contact at the County is Mr. Mich el <br /> Infurna (360/468-3454). <br /> EAI previously applied for a permit to install an off-site ground water monitoring well on City <br /> property. For convenience, we have enclosed a copy of that letter dated January 8, 1996 e <br /> permit for the well location proposed in our earlier application could not be granted due to he <br /> proximity of underground fibre-optic communication lines. <br /> EAI hereby requests the City to transfer our previous application materials, including e <br /> permit fees and the deposit to meet the City's bonding requirement, and apply those to a <br /> permit for this new location. Again, note that we are not installing a ground water monitorng <br /> well. We have enclosed another copy of our Certificate of Insurance adding the City as an <br /> additional insured. <br /> PLANNING,ENVIRONMENTAL ANALYSES AND <br /> HAZARDOUS SUBSTANCES MANAGEMENT AND REMEDIATION <br />
