<br /> 1 reviewed the Well Destruction Work Plan (Workplan), received 25 April 2011 and submitted by
<br /> Chevron's consultant AECOM. Additionally, two revised tables and a figure of the wells to be
<br /> destroyed was submitted by AECOM on 10 July 2011. The Workplan proposed the
<br /> decommissioning of eighty six monitoring wells (MW-1 through MW-7, MW-8D, MW-9R, MW-
<br /> 10, MW-10A, MW-11 through MW-14, MW-14A, MW-15R, MW-16 through MW-18, MW-18D,
<br /> MW-19S, MW-19D, MW-21 S, MW-21 D, MW-22S, MW-22D, MW-23S, MW-23D, MW-24S,
<br /> MW-24D, MW-25S, MW-25D, and MW-27 through MW-32) and remediation wells (VEW-1 A,
<br /> VEW-2A, VEW-3 through VEW-5, VEW-6A through VEW-13A , VEW-1 B, VEW-26, VEW-6B
<br /> through VEW-13B, VEW-16, VEW-19, VEW-22, SP-1 A, SP-1 B, SP-2 through SP-6, SP-7A,
<br /> SP-7B, SP-8, AS-14 through AS-18, AS-20, AS-21 , AS-26, AS-33 and SV-1 through SV-3) by
<br /> either pressure grouting (MW 2, MW-3, and MW-11) or over-drilling, and the removal of the soil
<br /> vapor extraction/air sparging remediation system.
<br /> The Workplan is approved with the condition that all surfaces be restored to surrounding
<br /> conditions (asphalt, concrete, soil, etc.). Please provide myself and the San Joaquin County
<br /> Environmental Health Department at least three working days notice prior to commencement of
